
Population Health Coach

- Develop and implement population health strategies and initiatives in collaboration with other healthcare professionals.
- Conduct individual and group health assessments to identify risk factors and create personalized health plans.
- Educate and empower individuals and communities on health promotion and disease prevention.
- Provide ongoing coaching and support to individuals in achieving their health goals.
- Collaborate with healthcare teams to coordinate care and resources for individuals with complex health needs.
- Utilize data and analytics to monitor and evaluate the effectiveness of population health programs.
- Stay current on population health trends, best practices, and evidence-based interventions.
- Build and maintain relationships with community organizations and resources to enhance population health efforts.
- Advocate for and promote health equity and social determinants of health in all population health initiatives.
- Maintain accurate and confidential records of population health interventions and outcomes.
- Participate in team meetings and trainings to continuously improve population health practices.
- Serve as a positive ambassador for Catholic Health Initiatives and its mission to provide compassionate care to all.
Bachelor's Degree In Healthcare-Related Field Or Equivalent Experience
Certified Health Coach (Chc) Or Similar Certification
Minimum Of 2 Years Of Experience In Population Health Management Or Related Field
Strong Knowledge Of Healthcare Industry, Including Current Trends And Issues
Excellent Communication And Interpersonal Skills, With The Ability To Build Rapport And Motivate Patients Towards Behavior Change.
