Job Description

At Catholic Health Initiatives, we are looking for an experienced, passionate Pharmacist to join our team. This person will be responsible for ensuring patient safety and providing the highest quality of care while meeting all regulatory and compliance requirements. The ideal candidate will be a self-driven professional with excellent problem-solving skills and a commitment to patient-centered care.The successful candidate will have a Bachelor’s degree in Pharmacy and be licensed to practice Pharmacy in the state where the job is located. They should have an understanding of clinical and therapeutic drug information and possess strong communication and customer service skills. The Pharmacist will have a thorough knowledge of the regulations and laws governing the practice of pharmacy and be highly organized and detail-oriented.Our ideal candidate will be a team player with excellent interpersonal skills and a dedication to patient safety. Responsibilities:

  1. Provide safe and effective pharmaceutical care to patients according to established protocols.
  2. Monitor drug therapies for safety, efficacy, and cost-effectiveness.
  3. Ensure the accuracy of prescriptions and maintain accurate records.
  4. Participate in pharmacy-related research and quality improvement initiatives.
  5. Maintain patient confidentiality in accordance with HIPAA regulations.
  6. Address patient inquiries regarding medications and provide appropriate counseling.
  7. Ensure compliance with all relevant laws and regulations.
  8. Develop and maintain positive relationships with external vendors and internal stakeholders.
  9. Compound and dispense medications as prescribed by physicians.
  10. Participate in clinical services such as medication therapy management and immunizations.
