Care Manager - Registered Nurse

We are looking for a highly professional and experienced Care Manager – Registered Nurse to join Catholic Health Initiatives. This is a great opportunity for a Registered Nurse looking to make a difference in the lives of our patients. In this role, you will be responsible for providing comprehensive care management services to our patients.We are looking for an individual who is compassionate, organized, and able to work well with both clinicians and patients. You must have a valid Registered Nursing license, current BLS and ACLS certification, and a minimum of two years of experience in a clinical setting. Preference is given to candidates with experience in a Care Management role. Experience with EMR systems is essential. The successful candidate will have excellent communication skills and the ability to communicate effectively with patients, families, and colleagues.
