  1. Coordinate and manage care for patients, ensuring timely and appropriate treatment plans are in place.
  2. Serve as a patient advocate, addressing their needs and concerns and advocating for their best interests.
  3. Collaborate with interdisciplinary teams to develop comprehensive care plans that meet the individual needs of patients.
  4. Ensure quality care is provided by monitoring and evaluating patient outcomes and making necessary adjustments to care plans.
  5. Communicate effectively with patients, their families, and other healthcare professionals to provide updates on treatment progress and address any concerns.
  6. Conduct thorough assessments of patients' medical history, current condition, and ongoing needs to inform care plans.
  7. Utilize electronic health records and other technology to accurately document patient information and track progress.
  8. Stay current on industry standards and best practices to continuously improve the quality of care provided.
  9. Adhere to all regulatory and organizational policies and procedures related to patient care.
  10. Demonstrate compassion, empathy, and professionalism in all interactions with patients and their families.

Job Qualifications
  • Bachelor's Degree In Nursing: The Candidate Should Have A Minimum Of A Bachelor's Degree In Nursing From An Accredited Institution. This Will Ensure That They Have A Strong Foundation In Nursing Principles And Patient Care.

  • Valid Rn License: The Candidate Must Possess A Valid And Current Rn License In The State Where They Will Be Practicing. This License Should Be In Good Standing And Should Not Have Any Restrictions.

  • Previous Care Coordination Experience: The Ideal Candidate Should Have At Least 2-3 Years Of Experience In Care Coordination, Preferably In A Healthcare Setting. This Will Demonstrate Their Ability To Effectively Manage Patient Care And Work With Interdisciplinary Teams.

  • Strong Communication And Organizational Skills: As A Care Coordinator, The Candidate Will Be Responsible For Communicating With Patients, Families, And Healthcare Professionals. Therefore, Excellent Communication And Organizational Skills Are Essential For This Role.

  • Knowledge Of Healthcare Regulations And Policies: The Candidate Should Have A Thorough Understanding Of Healthcare Regulations And Policies, Such As Hipaa And Medicare/Medicaid Guidelines. This Will Ensure That They Can Effectively Navigate The Complex Healthcare System And Advocate For Their Patients.
