- Collaborate with interdisciplinary teams to develop comprehensive care plans that meet the individual needs of patients.

Bachelor's Degree In Nursing: The Candidate Should Have A Minimum Of A Bachelor's Degree In Nursing From An Accredited Institution. This Will Ensure That They Have A Strong Foundation In Nursing Principles And Patient Care.
Valid Rn License: The Candidate Must Possess A Valid And Current Rn License In The State Where They Will Be Practicing. This License Should Be In Good Standing And Should Not Have Any Restrictions.
Previous Care Coordination Experience: The Ideal Candidate Should Have At Least 2-3 Years Of Experience In Care Coordination, Preferably In A Healthcare Setting. This Will Demonstrate Their Ability To Effectively Manage Patient Care And Work With Interdisciplinary Teams.
