Job Description

Catholic Health Initiatives is looking for an experienced Care Coordination Assistant to join our team in providing compassionate and quality care to our patients. We are seeking a professional who is highly organized and able to manage multiple tasks with attention to detail. The ideal candidate is a strong communicator and problem-solver with a passion for providing excellent patient care.The Care Coordination Assistant will be responsible for facilitating patient care plans and managing patient information. This position requires a thorough understanding of medical terminology, care coordination processes, and healthcare regulations.
