
Care Coordination Assistant
Catholic Health Initiatives is seeking an organized and detail-oriented Care Coordination Assistant to join our team. The ideal candidate will have excellent communication and problem-solving skills, and a commitment to providing excellent patient care. The Care Coordination Assistant will provide support to the Care Coordination team, helping to ensure that all patient needs are met in a timely and efficient manner.The successful candidate will possess a minimum of 1 year of experience in a medical office setting, and familiarity with medical terminology and insurance procedures. A high school diploma or GED equivalent is required, and an associate’s degree in a health-related field is preferred. Responsibilities:
- Support the Care Coordination team in ensuring that all patient needs are met in a timely and efficient manner.
- Communicate with patients and other medical professionals to ensure the best possible care is provided.
- Utilize excellent problem-solving and customer-service-oriented skills.
- Coordinate medical office activities, including insurance procedures and medical terminology.
- Maintain accurate patient records and other documents.
- Demonstrate strong organizational and multitasking skills.
- Follow all safety protocols and procedures.
- Provide additional assistance to other members of the Care Coordination team, as needed.
