Patient Care Coordinator

Bridge Home Healthcare & Hospice is seeking an experienced and compassionate Patient Care Coordinator to join our team. The ideal candidate for this role will be a highly organized individual with excellent communication skills and a passion for providing exceptional patient care. The Patient Care Coordinator is responsible for ensuring that our patients receive high quality, comprehensive care. Responsibilities:

• Develop and implement patient care plans • Maintain up-to-date records of patient progress and care plans • Ensure that all patient care guidelines and regulations are being followed • Respond to patient inquiries in a timely manner • Coordinate with other health care providers to ensure patient receives comprehensive care • Monitor patient care to ensure quality of service • Offer advice and support to patients and families • Educate patients and families about medical conditions and treatments • Facilitate referrals to other healthcare providers and services • Assist with marketing initiatives and outreach activities • Remain current on healthcare regulations and guidelines • Perform administrative tasks such as scheduling appointments and maintaining patient records
