
Patient Care Coordinator
Bridge Home Healthcare & Hospice is seeking an experienced and compassionate Patient Care Coordinator to join our team. The ideal candidate for this role will be a highly organized individual with excellent communication skills and a passion for providing exceptional patient care. The Patient Care Coordinator is responsible for ensuring that our patients receive high quality, comprehensive care. This position requires the ability to manage multiple tasks, remain flexible in a fast-paced environment, and maintain strong relationships with our patients and families.Qualifications:• Bachelor’s degree in Nursing, Healthcare Administration, or a related field• Previous experience in a healthcare setting preferred• Ability to build and maintain strong relationships with patients and families• Knowledge of current healthcare regulations and guidelines• Superior communication, interpersonal, and problem-solving skills• Excellent organizational and time management abilities• Proficiency in Microsoft Office and other relevant computer programs• A valid driver’s license and reliable transportationIf you have a passion for providing high-quality patient care and are looking for an opportunity to make a difference, we invite you to apply today!
Responsibilities:
• Develop and implement patient care plans • Maintain up-to-date records of patient progress and care plans • Ensure that all patient care guidelines and regulations are being followed • Respond to patient inquiries in a timely manner • Coordinate with other health care providers to ensure patient receives comprehensive care • Monitor patient care to ensure quality of service • Offer advice and support to patients and families • Educate patients and families about medical conditions and treatments • Facilitate referrals to other healthcare providers and services • Assist with marketing initiatives and outreach activities • Remain current on healthcare regulations and guidelines • Perform administrative tasks such as scheduling appointments and maintaining patient records
