Medical Social Worker

Welcome to Bridge Home Healthcare & Hospice! We’re thrilled that you’re interested in joining our team as a Medical Social Worker. We’re looking for an empathetic and knowledgeable individual to provide care and support to our patients and their families. In this role, you’ll be responsible for assessing the social and emotional needs of our patients and their families, providing counseling to those in need, facilitating referrals to appropriate social service programs, and helping to coordinate services. You’ll also be a vital part of our interdisciplinary team, working together with nurses, doctors, and other healthcare professionals to ensure the best possible outcomes for our patients.To be successful in this role, we’re looking for a candidate with a Master’s degree in social work from an accredited university and a valid license to practice social work in the state. Experience in home healthcare, hospice, or a related field is preferred but not required. Most importantly, we’re looking for someone who is compassionate, understanding, and has a passion for helping others.
