Director of Patient Care Services

We're seeking an experienced, compassionate, and dedicated Director of Patient Care Services to join our team at Bridge Home Healthcare & Hospice. This is a unique opportunity for a driven leader to make a positive impact on the lives of our patients and their families while managing a team of care professionals.The ideal candidate for this position will possess a strong clinical background and advanced knowledge of hospice and home healthcare services. They will have a proven track record of developing and leading multi-disciplinary care teams and will be an expert in delivering exceptional patient care. They must be a compassionate individual who takes pride in being an advocate for the patients and families we serve.The successful candidate will have a minimum of 5 years of related experience, a valid RN license, and a Bachelor's degree in Nursing, Healthcare Administration, or a related field. A Master's degree is preferred.

Responsibilities:

1. Lead and manage a multi-disciplinary team of care professionals to deliver exceptional patient care.
2. Develop patient care plans and ensure that patient and family needs are being met.
3. Monitor quality of care and recommend changes to improve outcomes.
4. Evaluate staff performance and provide mentoring and guidance.
5. Ensure compliance with all applicable regulations and standards.
6. Develop and implement strategies to reduce risk and improve efficiency.
7. Build relationships with other team members, physicians, and outside organizations.
8. Foster an environment of collaboration, communication and respect.
9. Advocate for the patients and families we serve.
10. Develop and present patient care plans to the organization's leadership.
