Admissions Registered Nurse (RN) - Hospice

The ideal candidate will be a Registered Nurse with a valid nursing license and a minimum of two years of experience in hospice or home health care. The Admissions Registered Nurse (RN) will be responsible for assessing patient's physical, psychosocial, emotional and spiritual needs, as well as providing guidance and support to family members. Responsibilities:

1. Assess patient's physical, psychosocial, emotional and spiritual needs.
2. Communicate effectively and efficiently with patients, families and other healthcare professionals.
3. Develop individualized care plans to meet the needs of each patient.
4. Provide guidance and support to family members.
5. Monitor patient progress and adjust care plans as needed.
6. Document patient assessments and care plans.
7. Maintain patient confidentiality.
8. Follow established policies, procedures and protocols.
9. Participate in educational activities and in-services.
10. Participate in quality improvement efforts.
