Job Description

BJC HealthCare is seeking a PRN RN Hospice Referral Coordinator to join our team and help us provide the highest quality of care to our patients. In this role, you will be responsible for coordinating hospice referrals and ensuring timely and appropriate care for patients.   1. Coordinate hospice referrals and ensure timely and appropriate care for patients.
  2. Communicate and collaborate with healthcare providers, patients, and their families to facilitate the referral process.
  3. Review and assess patient medical records to determine eligibility for hospice care.
  4. Educate patients and families on the hospice services and resources available.
  5. Schedule and coordinate hospice evaluations and admissions.
  6. Maintain accurate and up-to-date records of all hospice referrals and admissions.
  7. Collaborate with the hospice interdisciplinary team to develop and implement individualized care plans for patients.
  8. Monitor and track patient progress and communicate updates to the hospice team.
  9. Serve as a liaison between patients, families, and healthcare providers to ensure continuity of care.
  10. Stay current on hospice regulations, policies, and procedures to ensure compliance.
  11. Participate in quality improvement initiatives to enhance the hospice program.
  12. Provide emotional support and comfort to patients and families during the end-of-life journey.
  13. Maintain confidentiality and adhere to ethical standards in all interactions with patients and their families.
  14. Assist with training and mentoring new hospice referral coordinators.
  15. Continuously evaluate and improve processes to enhance the hospice referral and admission experience for patients and families.

Job Qualifications
  • Excellent Communication And Interpersonal Skills.

  • Bachelor's Degree In Nursing From An Accredited Institution.

  • Current Rn License In The State Of Missouri.

  • Minimum Of 2 Years Of Experience In Hospice Care Or Related Field.

  • Knowledge Of Hospice Referral And Admission Processes, As Well As Insurance And Reimbursement Procedures.
