Job Description

We are looking for an experienced Registered Nurse Case Manager (RN) to join BJC HealthCare. The ideal candidate should possess a compassionate and professional attitude, have a passion for providing excellent patient care, and be committed to our mission of providing exceptional patient experiences.The RN Case Manager will work as part of a multidisciplinary team to evaluate, coordinate, and manage patient care to ensure the highest quality of care. The successful candidate must have excellent clinical assessment skills, and a proven track record of providing exceptional patient care.We require the following qualifications: current Registered Nurse license for the state of Missouri, BSN or higher, three years of experience as an RN in a hospital or related setting, and a minimum of two years of experience in Case Management. Previous experience working with a variety of patients in a patient-centered environment is highly preferred. In addition, the RN Case Manager must be certified as a Case Manager (CCM) or obtain certification within 18 months of hire.

Responsibilities:

  1. Evaluate patient needs and develop individualized patient care plans.
  2. Coordinate patient care with other members of the multidisciplinary team.
  3. Monitor patient progress and adjust care plans as needed.
  4. Maintain accurate records of patient care and progress.
  5. Provide patient education and support to ensure optimal outcomes.
  6. Develop relationships with community resources to ensure patients receive necessary care.
  7. Advocate for patients to ensure their rights are respected.
  8. Respond to patient inquiries, concerns, and complaints in a timely manner.
  9. Remain up-to-date on current regulations, best practices, and evidence-based care.
  10. Maintain current Registered Nurse license for the state of Missouri, BSN or higher, and certification as a Case Manager (CCM) or obtain certification within 18 months of hire.
