Psych Rehab Specialist

1. Provide direct, compassionate care to individuals with mental health challenges.
2. Develop and implement individualized rehabilitation plans for patients based on their specific needs and goals.
3. Collaborate with other members of the healthcare team to ensure a holistic approach to patient care.
4. Monitor and document patient progress, making adjustments to treatment plans as needed.
5. Educate patients and their families on mental health disorders and the recovery process.
6. Facilitate group therapy sessions and provide individual counseling as needed.
7. Coordinate with community resources to ensure patients have access to necessary support services.
8. Maintain confidentiality and adhere to all ethical and legal standards.
9. Stay updated on current research and best practices in the field of mental health rehabilitation.
10. Foster a safe and supportive environment for patients to promote their recovery.
11. Attend and actively participate in team meetings and case conferences.
12. Communicate effectively with patients, their families, and other healthcare professionals.
13. Adhere to all policies and procedures of BJC HealthCare and maintain a high level of professionalism at all times.
14. Continuously evaluate and improve the quality of care provided to patients.
15. Represent BJC HealthCare and the Psych Rehab team positively in the community.
