  1. Serve as the primary point of contact for patients with complex medical needs, coordinating and managing their care within the Specialty Care Clinic.
  2. Collaborate with a multidisciplinary team of healthcare professionals to develop and implement patient care plans.
  3. Conduct thorough patient assessments and utilize clinical knowledge to provide high-quality and evidence-based care.
  4. Educate patients and their families on their medical conditions, treatments, and self-care strategies.
  5. Monitor patient progress and make necessary adjustments to care plans as needed.
  6. Maintain accurate and up-to-date patient records, including assessments, treatments, and medications.
  7. Serve as a resource and mentor to other nursing staff, providing guidance and support as needed.
  8. Communicate effectively with healthcare team members, patients, and their families to ensure continuity and coordination of care.
  9. Adhere to all hospital policies and procedures, as well as state and federal regulations.
  10. Participate in quality improvement initiatives and contribute to the overall success of the Specialty Care Clinic.

Job Qualifications
  • Current And Valid Registered Nurse (Rn) License In The State Of Missouri.

  • Minimum Of Bachelor's Degree In Nursing (Bsn) From An Accredited Institution.

  • Minimum Of 3-5 Years Of Experience In A Clinical Nursing Role, Preferably In A Specialty Care Clinic Setting.

  • Strong Leadership Skills And Experience In Managing A Team Of Nurses And Support Staff.

  • Knowledge And Understanding Of Specialty Care Procedures, Protocols, And Best Practices.
