- Provide compassionate care and support to patients and their families throughout their healthcare journey.
- Serve as a leader and role model for the Family Care Partner team, setting a high standard for patient-centered care.
- Communicate effectively with patients and their families, ensuring their needs and concerns are addressed in a timely and empathetic manner.
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate and facilitate patient care.
- Educate patients and families about their healthcare options, treatment plans, and resources available to them.
- Maintain accurate and detailed patient records, ensuring compliance with all confidentiality and privacy regulations.
- Identify and address any potential barriers to care, working to promote better health outcomes for patients and their families.
- Participate in continuous professional development activities to enhance knowledge and skills related to family care.
- Serve as an advocate for patients and their families, ensuring their voices are heard and their needs are met.
- Foster a positive and inclusive work environment, promoting teamwork and collaboration among the healthcare team.
- Adhere to all BJC HealthCare policies, procedures, and protocols to maintain a safe and efficient work environment.
- Act as a liaison between patients, families, and the healthcare team to facilitate open communication and resolve any issues that may arise.
- Continuously evaluate and improve upon processes and procedures to enhance the patient and family experience.
- Adhere to all regulatory and compliance standards related to family care, ensuring the highest quality of care is provided.
- Demonstrate a commitment to BJC HealthCare's mission and values, promoting a culture of excellence and service.
Bachelor's Degree In A Healthcare-Related Field Or Equivalent Experience.
Minimum Of 3 Years Experience In A Healthcare Setting, Preferably In Family Care Or Primary Care.
Strong Communication And Interpersonal Skills, Able To Effectively Collaborate With Patients, Families, And Healthcare Team Members.
Knowledge Of Electronic Medical Records And Ability To Navigate And Accurately Document Patient Information.
Ability To Provide Leadership And Guidance To Fellow Family Care Partners, Including Delegating Tasks And Resolving Conflicts.
