
Community Support Specialist

- Conduct needs assessments: The Community Support Specialist will conduct thorough needs assessments with patients to understand their specific needs and determine the appropriate resources and support services to connect them with.

Excellent Communication And Interpersonal Skills.
Bachelor's Degree In Social Work, Psychology, Or A Related Field.
Experience Working With Individuals With Mental Health And/Or Developmental Disabilities.
Knowledge Of Community Resources And Ability To Connect Individuals To Appropriate Services.
