Community Support Specialist

We are seeking a Community Support Specialist to join BJC HealthCare and help us to create healthy communities for our patients and their families. Responsibilities:

1. Develop and maintain relationships with community health partners and stakeholders.
2. Design and implement community health initiatives that promote healthy behaviors and improved health outcomes.
3. Educate the community on BJC HealthCare's mission and values.
4. Coordinate and lead health education programs and activities.
5. Develop and manage program budgets and resources.
6. Monitor program outcomes and provide reports to management.
7. Develop and implement plans to address gaps in community health resources.
8. Coordinate with other departments and staff to ensure the successful implementation of community health programs.
9. Provide support to community health partners and stakeholders.
10. Analyze data and trends to identify areas of improvement and develop strategies to address them.
