- Develop and maintain relationships with community organizations, healthcare providers, and other stakeholders to coordinate services and resources.
- Conduct outreach and education to raise awareness about available healthcare services and resources.
- Assist individuals and families in navigating the healthcare system and accessing needed services.

- Serve as a liaison between the community and healthcare providers to improve communication and understanding.
- Maintain confidentiality and adhere to ethical standards in all interactions with individuals and families.
- Adhere to all policies, procedures, and regulations related to community health work.
- Continuously evaluate and assess the effectiveness of community health programs and make recommendations for improvement.
- Act as an advocate for the importance of community health and wellness.
- Utilize effective communication skills to build rapport and establish trust with individuals and families.
- Collaborate with other team members to ensure coordinated and comprehensive care for individuals and families.
- Keep up-to-date with current trends and developments in the field of community health work.

Bachelor's Degree In Public Health, Community Health, Or A Related Field
Certification As A Community Health Worker Or Willingness To Obtain Certification Within First Year Of Employment
Minimum Of 2 Years Of Experience Working In A Community Health Setting
Strong Communication And Interpersonal Skills, With The Ability To Effectively Engage And Build Relationships With Diverse Populations
Knowledge Of Local Resources And Services For Underserved Populations, Including Experience Connecting Individuals To These Resources.
