- Assist community members in navigating the healthcare system and accessing appropriate resources and services.
- Collaborate with healthcare professionals to ensure coordinated care for community members.
- Provide culturally competent and compassionate support to individuals and families from diverse backgrounds.
- Organize and facilitate health education workshops and events for community members.
- Monitor and track progress of community members' health goals and make necessary referrals.
- Maintain accurate and confidential records of interactions and services provided.
- Stay informed about current health issues and resources in the community and share this information with community members.
- Advocate for policies and programs that promote overall community health and well-being.
- Participate in team meetings and trainings to enhance skills and knowledge related to community health work.
- Represent the organization in a professional and positive manner at community events and meetings.
- Regularly report on activities, outcomes, and challenges to superiors.
- Adhere to ethical standards and guidelines for community health work.
Bachelor's Degree In Public Health, Social Work, Or A Related Field.
Previous Experience Working In A Community Health Setting Or With Underserved Populations.
Strong Communication And Interpersonal Skills To Effectively Engage With Diverse Communities.
Knowledge Of Public Health Principles And Practices, Including Health Education And Disease Prevention Strategies.
Ability To Work Independently And As Part Of A Multidisciplinary Team To Coordinate And Implement Community Health Programs And Initiatives.
