Job Description

BayCare Health System is looking for an experienced and compassionate Social Services Specialist to join our team. In this role, you will assist in providing a comprehensive social service program for patients and their families to ensure that they are receiving the best care possible. The ideal candidate for this position will have a Bachelor’s degree in Social Work or a related field, excellent communication skills, and experience working in a healthcare setting. They will be a team player who is organized, compassionate, and passionate about helping those in need. Responsibilities:

  1. Develop and maintain relationships with patients and their families to provide social services and resources.
  2. Assist patients and their families in navigating the healthcare system, identifying social service resources, and creating individualized care plans.
  3. Conduct assessments of patient and family needs to determine the most appropriate services and resources.
  4. Provide ongoing support and counseling to patients and their families.
  5. Collaborate with healthcare providers to ensure a comprehensive approach to patient care.
  6. Maintain accurate documentation of all patient interactions and services provided.
  7. Promote awareness of available community resources to patients and their families.
  8. Advocate for patients and their families to ensure their needs are met.
  9. Participate in meetings and training sessions as required.
  10. Adhere to all policies and protocols of the organization.
