  13. Serve as a resource for patients and their families, providing education and support on healthcare resources and services.

Job Qualifications
  • Bachelor's Degree In Nursing, Social Work, Or Related Healthcare Field.

  • Minimum Of 3 Years Of Experience In Case Management Or Care Coordination Role In A Hospital Setting.

  • Strong Knowledge Of Healthcare Regulations, Insurance Plans, And Reimbursement Processes.

  • Excellent Communication And Interpersonal Skills To Effectively Collaborate With Patients, Families, And Healthcare Team Members.

  • Current Certification In Case Management, Such As Ccm Or Acm, Preferred.
