RN Pediatric Case Manager

The ideal candidate for this role will have a minimum of two years' experience working as a Pediatric RN Case Manager, a valid RN license, and the ability to work collaboratively with physicians, families, and members of the interdisciplinary team. The RN Pediatric Case Manager will be responsible for developing, implementing, and monitoring comprehensive plans of care for pediatric patients in accordance with the guidelines and regulations of the governing authorities. Responsibilities:

1. Develop, implement, and monitor comprehensive plans of care for pediatric patients in accordance with the guidelines and regulations of the governing authorities.
2. Establish and maintain collaborative relationships with physicians, families, and members of the interdisciplinary team.
3. Provide quality home health care services to pediatric patients and their families.
4. Assist in evaluating the effectiveness of care and developing new strategies to improve care.
5. Communicate with patients, physicians, and other members of the care team regarding patient care and progress.
6. Maintain accurate documentation of patient care and progress.
7. Participate in in-service training and continuing education opportunities.
8. Adhere to the standards of ethical, professional, and legal nursing practice.
9. Ensure compliance with all applicable laws, regulations, and policies.
10. Perform other duties as assigned.
