Occupational Therapist - Home Health Visits

As an Occupational Therapist, you will be responsible for providing direct patient care to clients in their homes. We value our therapists and the expertise they bring to our team. Responsibilities: • Provide direct patient care to clients in their homes • Collaborate with a multidisciplinary team of healthcare professionals to ensure the highest quality of care for clients • Develop individualized treatment plans for each patient • Monitor patient progress toward established goals • Educate patients and their families on therapeutic techniques • Document patient progress in medical records • Ensure all treatments are compliant with state and federal regulations • Communicate with other health care providers to coordinate care • Maintain a safe and clean environment for client visits • Foster positive relationships with patients and their families • Assist in training and mentoring new staff members, if needed
