Job Description

At BAYADA Home Health Care, we are looking for a compassionate and highly organized individual to join our team as a Hospice Case Manager. This role requires an individual who is passionate about providing excellent care and support to hospice patients and their families. The Hospice Case Manager will be responsible for providing care management, coordination, and support for a caseload of hospice patients and their families throughout their hospice journey.The successful candidate will have a Bachelor’s Degree in Nursing or a related field and a valid Registered Nurse (RN) license in the state of practice. A minimum of two (2) years of experience in home health or hospice care is required, as well as knowledge of hospice regulations and standards of practice. The ideal candidate will possess excellent communication, organizational, and problem-solving skills, as well as a compassionate and patient-centered attitude.If you have a passion for providing excellent care and support to hospice patients and their families, we encourage you to apply for this rewarding and challenging role.

Responsibilities:

  1. Provide care management, coordination, and support for a caseload of hospice patients and their families throughout their hospice journey.
  2. Ensure all care is provided in accordance with hospice regulations and standards of practice.
  3. Develop and implement plans of care for hospice patients in collaboration with their families, healthcare providers, and other members of the interdisciplinary team.
  4. Monitor patient progress and adjust care plans as needed to ensure quality of care.
  5. Ensure patients and families are receiving the necessary emotional and spiritual support.
  6. Act as a resource for patients, families, caregivers, and other members of the healthcare team in order to provide support during the hospice journey.
  7. Participate in interdisciplinary team meetings and other hospice-related activities.
  8. Document patient care and services in an accurate and timely manner.
  9. Assist with patient admissions and discharges, as necessary.
  10. Participate in continuing education and professional development activities.

Job Qualifications
  • Bachelor's Degree In Healthcare Related Field
