
Clinical Pharmacist – Hematology Oncology
At Ambercare, we are looking for an experienced and passionate Clinical Pharmacist – Hematology Oncology to join our team. The ideal candidate is a board certified pharmacist with great interpersonal and problem-solving skills who can provide comprehensive medication management services to patients with cancer and blood disorders. Required qualifications include a Doctor of Pharmacy degree from an accredited institution, current licensure in the state of practice, and completion of a clinical residency or fellowship in oncology or hematology/oncology. Candidates must possess exceptional knowledge of oncology and hematology medications, including appropriate dosing, adverse effects, and drug interactions. In addition, the successful candidate must have excellent communication and organizational skills, as well as the ability to work independently and as part of a multidisciplinary health care team.
Responsibilities:
- Provide comprehensive medication management services to patients with cancer and blood disorders.
- Maintain current knowledge of oncology and hematology medications, including appropriate dosing, adverse effects, and drug interactions.
- Develop individualized medication management plans and provide patient education.
- Monitor patient responses to medications and adjust as necessary.
- Collaborate with other health care professionals to optimize patient care.
- Educate health care providers on medication use and best practices.
- Participate in quality improvement activities to ensure patient safety.
- Maintain accurate and up-to-date patient records.
- Participate in the development and implementation of pharmacy policies and procedures.
- Adhere to all regulatory requirements and laws related to pharmacy practice.
Bachelor's Degree In Pharmacy Or Related Field
+ Years' Experience In A Clinical Pharmacy Setting
+ Years' Experience In Hematology/Oncology
Current Pharmacist License
Board Certification In Oncology Pharmacy
Knowledge Of Chemotherapy
