Ambercare is looking for an experienced and compassionate Clinical Care Coordinator to join our team. The Clinical Care Coordinator’s primary responsibility is to provide comprehensive care management to our patients, including assessing patient needs, developing plans of care, managing the delivery of services, and monitoring outcomes. We’re looking for a motivated individual who is committed to providing excellent patient care and enhancing the lives of our patients.The ideal candidate for this position has a Bachelor's degree in a relevant field and a minimum of two years of experience in a health care setting. They should also possess excellent communication, problem-solving, and organizational skills. Additionally, the successful candidate must be passionate about providing outstanding patient care and have a desire to make a difference in people’s lives. Responsibilities:

  1. Develop and implement comprehensive care plans for patients, taking into account their individual needs, preferences, and goals.
  2. Monitor and assess the effectiveness of services and treatments provided to patients.
  3. Maintain a high level of communication with patients and their families, providing support, education, and advocacy.
  4. Coordinate with other healthcare professionals to ensure the delivery of quality care.
  5. Develop and maintain relationships with community-based organizations and resources to provide referrals as needed.
  6. Document patient care services in accordance with organizational policy and procedures.
  7. Ensure compliance with relevant laws, regulations, and standards.
  8. Participate in regular team meetings to discuss patient care and other relevant topics.
  9. Adhere to safety protocols and procedures.
  10. Remain current with advances in healthcare and clinical care management.
