Waiver Case Manager

We are looking for an experienced and dedicated Waiver Case Manager to join our team at Allina Health System. The ideal candidate will have a passion for helping others and a commitment to providing quality care. This individual will be responsible for providing comprehensive case management services to adults with disabilities and mental health issues.The successful candidate must have a Bachelor’s degree in Social Work, Psychology, or related field and a minimum of two years of case management experience with adults with disabilities and/or mental health issues. Knowledge of the disability waiver programs and eligibility requirements is also required. A valid driver’s license and access to a reliable vehicle are also necessary.The Waiver Case Manager will be a compassionate advocate for their clients and must be able to work collaboratively with a variety of stakeholders to ensure clients receive the most appropriate services. This individual must possess excellent communication skills, be well organized, and have a strong attention to detail.
