4. Connect patients with community resources and support programs to enhance their social support network and improve their access to necessary services.
5. Advocate for patients' rights and needs, working to ensure they receive appropriate care and support.
6. Document all interactions with patients and maintain accurate records of services provided.
7. Stay up-to-date with current social work practices and trends to provide the best possible care to patients.
8. Collaborate with other social workers and healthcare professionals to develop and implement programs and initiatives to improve patient outcomes.
9. Participate in team meetings, case conferences, and trainings to share knowledge and expertise with colleagues.
10. Maintain confidentiality and adhere to ethical standards when working with patients and their families.
11. Adhere to all organizational policies and procedures, as well as state and federal laws and regulations.
12. Continuously evaluate and assess the effectiveness of interventions and make adjustments as necessary.
13. Provide crisis intervention and support to patients and families in emergency situations.
14. Communicate effectively and compassionately with patients and their families, demonstrating empathy and understanding.
15. Serve as a liaison between patients, families, and healthcare providers to facilitate effective communication and collaboration.
