RN Case Manager Hospice

The RN Case Manager will be responsible for providing end-of-life care to patients and their families in a compassionate and supportive manner.We are seeking a candidate who has a passion for patient care and a commitment to providing excellent service. The ideal candidate should have a current nursing license in the state of Minnesota and two years of experience working in hospice or home health care. Knowledge of hospice philosophy, disease processes, and symptom management is essential.
