- Dispensing and monitoring medications for inpatient and infusion patients according to established protocols and guidelines.
- Collaborating with healthcare providers to ensure safe and effective medication therapy.
- Conducting medication regimen reviews and making appropriate recommendations.
- Maintaining accurate and complete patient records, including medication profiles, allergies, and drug interactions.
- Providing patient education on medication usage, side effects, and drug interactions.
- Participating in quality improvement initiatives to enhance patient care.
- Ensuring compliance with all state and federal laws and regulations related to medication dispensing and management.
- Overseeing and managing medication inventory to ensure adequate supply and proper storage.
- Collaborating with pharmacy technicians to ensure efficient and accurate medication dispensing.
- Staying updated on current trends and developments in pharmacy practice and incorporating them into daily work.
- Providing excellent customer service to patients, families, and healthcare team members.
- Participating in multidisciplinary rounds and communicating with other healthcare providers to optimize patient care.
- Maintaining a clean, organized, and safe work environment.
- Assisting with training and mentoring new pharmacy staff.
- Adhering to all hospital policies and procedures, including those related to safety, confidentiality, and infection control.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy Program.
Active And Unrestricted Pharmacy License In The State Of Employment.
Experience In Inpatient And Infusion Pharmacy Settings, Including Knowledge Of Iv Medication Preparation And Administration.
Strong Clinical Knowledge And Understanding Of Pharmacokinetics, Drug Interactions, And Medication Management In Hospitalized Patients.
Excellent Communication And Teamwork Skills, With The Ability To Collaborate Effectively With Physicians, Nurses, And Other Healthcare Professionals In A Fast-Paced, Multidisciplinary Environment.
