1. Provide direct occupational therapy services to patients in their homes, following best practices and utilizing evidence-based techniques.
2. Create individualized treatment plans based on patient evaluations and ongoing assessments.
3. Collaborate with other members of the healthcare team, including physicians, nurses, and social workers, to ensure comprehensive and coordinated care.
4. Educate patients and their families on strategies for improving independence, mobility, and daily living skills.
5. Maintain accurate and timely documentation of patient progress, treatments, and outcomes.
6. Adhere to all regulatory and professional standards, including HIPAA guidelines.
7. Continuously evaluate and modify treatment plans to ensure optimal patient outcomes.
8. Provide guidance and support to certified occupational therapy assistants, as needed.
9. Participate in interdisciplinary team meetings and contribute to the development of patient care plans.
10. Stay current with advancements in occupational therapy practices and techniques through continuing education and professional development opportunities.
11. Advocate for the needs of patients and their families, and provide resources and referrals as necessary.
12. Communicate effectively with patients, families, and caregivers, and maintain a compassionate and professional demeanor at all times.
