Mental Health Coordinator

The Mental Health Coordinator will be responsible for helping to create and maintain a positive environment for patients and staff. The ideal candidate should have a minimum of two years of mental health experience, as well as a strong background in working with individuals in both acute and outpatient settings. In addition, they should be passionate about making a positive impact on the mental health of our patients and staff. Responsibilities:

1. Develop and implement individualized treatment plans for patients in accordance with the Allina Health System's standards and policies.
2. Monitor patient progress and evaluate outcomes of treatment plans.
3. Provide support and guidance to patients and their families to help them cope with mental health challenges.
4. Develop and coordinate support programs for patients and staff.
5. Liaise with other health care professionals to ensure necessary services are provided to patients.
6. Monitor and evaluate the effectiveness of mental health services.
7. Develop and maintain positive relationships with patients, families, and staff.
8. Serve as an advocate for patients in need of mental health services.
9. Remain current on best practices and emerging trends in the mental health field.
10. Collaborate with multidisciplinary teams to ensure the best possible outcomes for patients.
