  1. Provide high-quality and compassionate nursing care to patients in their homes.
  2. Conduct thorough assessments of patients' physical, emotional, and social needs.
  3. Develop individualized care plans based on patients' needs and goals.
  4. Administer medications, treatments, and procedures as prescribed by physicians.
  5. Educate patients and their families about their conditions, treatments, and self-care.
  6. Monitor and evaluate patients' progress and adjust care plans as needed.
  7. Collaborate with other healthcare professionals to ensure coordinated and comprehensive care.
  8. Maintain accurate and timely documentation of all patient care activities.
  9. Adhere to all regulatory standards and best practices in home healthcare.
  10. Serve as an advocate for patients and their families, promoting their rights and needs.
  11. Provide emotional support and counseling to patients and their families.
  12. Communicate effectively with patients, families, and caregivers to ensure understanding and compliance with care plans.
  13. Act as a resource and mentor for other team members, sharing knowledge and expertise.
  14. Participate in continuing education and professional development to maintain and enhance skills and knowledge.
  15. Maintain confidentiality and respect patients' privacy at all times.
  16. Comply with all organizational policies and procedures.
  17. Demonstrate a commitment to providing exceptional patient-centered care.
  18. Travel to patients' homes and maintain a flexible schedule to accommodate patients' needs.
  19. Demonstrate a positive and professional attitude towards patients, families, and coworkers.

Job Qualifications
  • Active Rn License In The State Of Minnesota: The Candidate Must Possess A Valid And Current License To Practice As A Registered Nurse In The State Of Minnesota, As Well As Any Necessary Certifications And Credentials.

  • Minimum Of 2 Years Of Home Care Nursing Experience: Allina Health System Is Seeking An Experienced Rn With At Least 2 Years Of Experience In Home Care Nursing. This Experience Will Ensure The Candidate Is Familiar With The Unique Challenges And Responsibilities Of Providing Care In A Patient's Home.

  • Strong Clinical Assessment And Critical Thinking Skills: The Home Care Visit Rn Will Be Responsible For Assessing Patients' Needs, Developing Care Plans, And Making Clinical Judgments. Therefore, Strong Clinical Assessment And Critical Thinking Skills Are Essential For Success In This Role.

  • Excellent Communication And Interpersonal Skills: The Candidate Must Possess Strong Communication And Interpersonal Skills To Effectively Communicate With Patients, Families, And Other Healthcare Team Members. This Includes The Ability To Provide Education, Support, And Guidance To Patients And Their Caregivers.

  • Ability To Work Independently And Manage Time Effectively: As A Home Care Visit Rn, The Candidate Will Be Required To Work Independently And Manage Their Time Effectively. This Includes The Ability To Prioritize Tasks, Meet Deadlines, And Adapt To Changing Patient Needs. Previous Experience With Home Care Documentation Systems Is Also Preferred.
