- Provide skilled nursing services to patients of all ages in their homes
- Conduct thorough assessments of patients' health status and create individualized care plans
- Administer medications, treatments, and procedures as prescribed by physicians
- Monitor and document patients' progress and report any changes to the healthcare team
- Educate patients and their families on disease management, medication and treatment plans, and home care best practices
- Collaborate with other healthcare professionals to ensure coordinated and comprehensive care for patients
- Maintain accurate and timely documentation of all patient interactions, treatments, and progress
- Adhere to all regulatory standards and guidelines for home care services
- Communicate effectively with patients, families, and healthcare team members to ensure a positive and supportive care experience
- Participate in ongoing training and development opportunities to enhance clinical skills and knowledge
- Act as an advocate for patients, ensuring their needs and preferences are communicated and addressed in their care plan.
Valid Rn License In The State Of Minnesota
Minimum Of 1 Year Of Experience In Home Care Or Community Health Nursing
Strong Clinical Skills And Knowledge Of Current Nursing Practices
Ability To Work Independently And Manage A Caseload Of Diverse Patient Populations
Excellent Communication And Interpersonal Skills To Effectively Collaborate With Patients, Caregivers, And Interdisciplinary Team Members.
