Home Care Physical Therapist

Responsibilities:

1. Evaluate patient needs and develop individual treatment plans to improve mobility, reduce pain, and aid in physical recovery.

4. Monitor patient progress and adjust treatment plans as needed.
5. Work with multidisciplinary teams to coordinate patient care.
6. Maintain accurate and up-to-date patient records.
7. Provide patient and family education on physical therapy treatments and safety.
8. Assist in the development and implementation of patient care plans.
9. Collaborate with other healthcare professionals to ensure the most effective patient care.
10. Follow all safety regulations and adhere to established policies and procedures of the organization.
