Home Care Occupational Therapist

Allina Health System is looking for an experienced and passionate Home Care Occupational Therapist to join our growing team. We are looking for a highly motivated and organized individual who can provide the highest-quality care for our patients. The Home Care Occupational Therapist will be responsible for planning, organizing and administering individualized treatment programs for patients with physical, mental, and/or emotional impairments, to restore, reinforce, and enhance their performance in activities of daily living. The ideal candidate will have a Master's Degree in Occupational Therapy, be licensed in the state of Minnesota, and have at least one year of experience in a home care setting. Responsiblities:

1. Develop and administer individualized treatment plans for patients with physical, mental, and/or emotional impairments.

3. Develop and implement patient/family education programs.
4. Monitor patient progress and adjust treatments accordingly.
5. Maintain accurate and detailed patient records.
6. Collaborate with other health professionals to ensure the best possible care for the patient.
7. Adhere to relevant regulations and guidelines related to occupational therapy.
8. Participate in professional development activities to stay up-to-date with best practices.
9. Promote a positive and supportive working environment.
10. Utilize excellent communication and interpersonal skills to build rapport with patients and their families.
