We are searching for a skilled and dedicated Community Palliative Care RN to join our team and provide exceptional care to our patients and their families. The ideal candidate for this role is a skilled nurse who is experienced in palliative care and has strong interpersonal and communication skills. Required Qualifications: • Registered Nurse (RN) license in the State of Minnesota • 3-5 years of nursing experience with at least 2 years in palliative care• Ability to work independently and as part of a team • Excellent communication and listening skills • Ability to work effectively with patients, families, and other healthcare professionals • Knowledge of palliative care standards and regulations • Ability to demonstrate sound clinical judgment • Compassionate nature and excellent bedside manner • Ability to work in a fast-paced environment • BLS and ACLS certification

Responsibilities:

  1. Provide exceptional palliative care to patients and families.
  2. Act as a liaison between patients, their families, and other healthcare professionals.
  3. Maintain up-to-date knowledge of palliative care standards and regulations.
  4. Demonstrate sound clinical judgment when making decisions.
  5. Exercise compassion and a good bedside manner with patients and families.
  6. Work in a fast-paced environment while providing quality care.
  7. Document patient care and other related information.
  8. Maintain BLS and ACLS certification.
  9. Adhere to all safety protocols and infection control procedures.
  10. Provide patient and family education and support.
