Lead Case Manager

Provides operational supervision of Case Managers and HDM Coordinators. Is available to provide consultation, guidance, direction, resource identification, and problem resolution, seeking input from Manager as necessary. Assists in establishing and monitoring procedures which ensure compliance to Older American Act and Community Meals on Wheels OC policies and procedures as pertained to HDM, In Home, and Case Management programs. Leads all training efforts for the Social Services Department, including training of new hires, interns and case managers. Oversees and acts as the lead for internship and volunteers rotating through the Social Services department. Maintains a case load with a special focus on difficult clients. Leads and oversees the case management meetings which includes case consultations, resource management. Acts as the liaison for other departments and outside agencies in communication and services coordination.
