  1. Coordinate and manage patient care in their homes, ensuring all medical needs are met and treatments are followed.
  2. Work directly with patients to understand their individual needs and develop personalized care plans.
  3. Maintain thorough and accurate patient records, including medical history, treatment plans, and progress notes.
  4. Communicate effectively with patients, their families, and healthcare providers to ensure continuity of care and address any concerns or issues.
  5. Conduct home visits and assessments to determine the best course of care for each patient.
  6. Provide education and support to patients and their families, helping them to understand treatment plans and navigate the healthcare system.
  7. Collaborate with interdisciplinary teams and healthcare providers to ensure the best possible outcomes for patients.
  8. Remain up-to-date on industry trends and best practices in patient care coordination.
  9. Develop and maintain strong relationships with patients, their families, and healthcare providers.
  10. Adhere to all legal and ethical standards and maintain patient confidentiality at all times.

Job Qualifications
  • Bachelor's Degree In Nursing Or A Related Healthcare Field.

  • Minimum Of 2 Years Of Experience In A Clinical Setting, Preferably In Patient Coordination Or Case Management.

  • Strong Communication And Interpersonal Skills To Effectively Interact With Patients, Healthcare Professionals, And Insurance Providers.

  • Proficiency In Electronic Medical Record Systems And Other Healthcare Technology.

  • Knowledge Of Healthcare Regulations And Compliance Standards, Including Hipaa And Medicare Guidelines.
