Job Description

Providence St. Patrick Hospital is seeking a passionate and experienced Care Manager RN to join our team. The Care Manager RN will be responsible for providing comprehensive assessment, care planning, and coordination of services to ensure that patients receive the highest quality of care. This position requires excellent communication and organizational skills, as well as the ability to work collaboratively with a multi-disciplinary team.The ideal candidate for this role is a registered nurse with a current RN license in the state of Montana and at least two years of experience in a care management role. The successful candidate will be highly organized and be able to manage multiple projects at once while delivering exceptional care to our patients. They must also have a customer-focused mindset and be an enthusiastic team player. Knowledge of health care regulations and reimbursement processes are a plus.

Responsibilities for Care Manager RN:

  1. Conduct comprehensive assessments and create individualized care plans for patients.
  2. Coordinate services between multiple disciplines to ensure high quality care is provided.
  3. Utilize excellent communication and organizational skills to manage multiple projects.
  4. Provide customer-focused care to patients and their families.
  5. Demonstrate an enthusiastic attitude and the ability to collaborate with a multidisciplinary team.
  6. Maintain current RN license in the state of Montana.
  7. Possess a thorough understanding of healthcare regulations and reimbursement processes.
  8. Investigate and identify potential improvement opportunities within the care management process.
  9. Monitor and track patient progress to ensure that necessary care is provided in a timely manner.
  10. Develop and maintain strong relationships with patients, families, and other healthcare professionals.
