  1. Coordinate and manage the care of patients throughout their hospital stay.
  2. Advocate for the needs and well-being of patients with healthcare providers.
  3. Conduct thorough assessments of patients' medical, emotional, and social needs.
  4. Develop and implement individualized care plans for patients in collaboration with healthcare team.
  5. Monitor and evaluate the progress and effectiveness of care plans.
  6. Communicate regularly with patients, their families, and healthcare team to ensure coordinated and comprehensive care.
  7. Provide education and resources to patients and their families to support their health and recovery.
  8. Keep detailed and accurate records of patient care and progress.
  9. Collaborate with insurance companies and other agencies to ensure proper coverage and resources for patients.
  10. Participate in interdisciplinary team meetings to discuss and review patient cases.
  11. Stay up-to-date with healthcare regulations and policies.
  12. Maintain patient confidentiality and adhere to HIPAA regulations.
  13. Demonstrate empathy and compassion towards patients and their families.
  14. Act as a liaison between the hospital, patients, and their families.
  15. Identify and address any barriers or challenges to patient care.
  16. Support and promote a positive working environment within the hospital.
  17. Participate in the development and implementation of quality improvement initiatives.
  18. Attend trainings and conferences to enhance knowledge and skills.
  19. Assist with discharge planning and post-hospital care arrangements.
  20. Adhere to ethical and professional standards in all aspects of job performance.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work, Nursing, Or Related Field.

  • Minimum Of 2 Years Experience In Case Management Or Related Healthcare Field.

  • Knowledge Of Healthcare Systems, Insurance Policies, And Medical Terminology.

  • Strong Communication And Interpersonal Skills, With The Ability To Build Rapport And Collaborate With Patients, Families, And Multidisciplinary Teams.

  • Ability To Assess Patient Needs, Develop Individualized Care Plans, And Provide Appropriate Resources And Referrals.
