  1. Provide compassionate care to patients in their homes: The LPN will be responsible for visiting patients in their homes and providing compassionate care, including administering medication, dressing wounds, and monitoring vital signs.
  2. Collaborate with interdisciplinary team: The LPN will work closely with other healthcare team members, including physicians, therapists, and social workers, to develop and implement individualized care plans for patients.
  3. Maintain accurate patient records: The LPN will be responsible for documenting all patient care activities accurately and in a timely manner, ensuring that all records comply with legal and ethical standards.
  4. Assess patient needs: The LPN will perform initial and ongoing assessments of patients to determine their physical, emotional, and social needs and provide appropriate care accordingly.
  5. Educate patients and families: The LPN will educate patients and their families on disease processes, medications, and self-care techniques to promote better health and independence.
  6. Ensure patient safety: The LPN will follow all safety protocols and procedures to ensure the safety of patients in their homes and report any potential safety hazards.
  7. Communicate effectively: The LPN will communicate effectively with patients, families, and healthcare team members to provide updates on patient progress and address any concerns or issues.
  8. Adhere to standards of practice: The LPN will adhere to all standards of practice, policies, and procedures set by the organization and regulatory bodies.
  9. Continuously improve skills and knowledge: The LPN will participate in ongoing education and training to maintain and improve their skills and knowledge, staying up-to-date with best practices in home health care.
  10. Maintain professionalism: The LPN will maintain a professional demeanor at all times, building trust and rapport with patients and their families, and representing the organization positively in the community.

Job Qualifications
  • Active Lpn Licensure: The Candidate Must Hold A Current And Active Lpn License In The State Where The Job Is Located.

  • Minimum Of 2 Years Of Clinical Experience: Humana Requires Applicants To Have A Minimum Of 2 Years Of Clinical Experience, Preferably In A Home Health Or Long-Term Care Setting.

  • Strong Communication And Interpersonal Skills: As A Home Health Lpn, The Candidate Will Be Working Closely With Patients, Their Families, And Other Healthcare Professionals. Therefore, Strong Communication And Interpersonal Skills Are Essential For This Role.

  • Knowledge Of Medicare And Medicaid Regulations: Humana Is A Medicare And Medicaid Provider, So Knowledge Of These Regulations Is Important For An Lpn Working In Home Health.

  • Ability To Work Independently: Home Health Lpns Are Often Responsible For Managing Their Own Caseload And Providing Care In Patients' Homes. Therefore, The Candidate Must Be Able To Work Independently And Have Strong Time Management And Organizational Skills.
