Behavioral Health Case Manager

Elevance Health is seeking a passionate, experienced, and detail-oriented Behavioral Health Case Manager to join our team. In this role, you will provide comprehensive case management services to individuals and their families who are struggling with mental health and/or substance abuse issues. You will ensure that clients have access to necessary resources, develop service plans, and coordinate with other providers to ensure the best care for your clients.The ideal candidate for this position will have a Master’s degree in Psychology, Social Work, or a related field and at least one year of experience providing case management services, preferably in a behavioral health setting. Additionally, you must have strong communication and organizational skills, as well as the ability to work both independently and as part of a team. You should be compassionate and have a deep understanding of the challenges and needs of individuals and families affected by mental health and/or substance abuse issues. Responsibilities:

2. Ensure that clients have access to necessary resources.
3. Develop service plans.

5. Maintain strong communication and organizational skills.

8. Monitor and document client progress.
9. Provide crisis intervention services as needed.
10. Develop and maintain relationships with community resources and referral sources.
