As a Hospitalist NP, you will play a vital role in managing and coordinating the care of hospitalized patients, working closely with physicians and other healthcare professionals to ensure the best possible outcomes.   1. Provide comprehensive and compassionate care to hospitalized patients in collaboration with physicians and other healthcare professionals.
  2. Conduct thorough patient assessments and develop individualized treatment plans based on patient needs and medical history.
  3. Monitor and manage patient progress, including ordering and interpreting diagnostic tests, adjusting medications, and coordinating consultations with specialists.
  4. Communicate effectively with patients, families, and healthcare team members to ensure a coordinated and patient-centered approach to care.
  5. Educate patients and families on disease management, treatment plans, and preventive measures to promote optimal health outcomes.
  6. Adhere to evidence-based practice guidelines, facility policies, and regulatory standards to ensure safe and effective patient care.
  7. Participate in interdisciplinary rounds and case conferences to discuss patient progress, discharge planning, and care coordination.
  8. Collaborate with case managers, social workers, and other support staff to ensure seamless transitions of care and appropriate discharge planning.
  9. Maintain accurate and timely documentation of patient care, including progress notes, treatment plans, and discharge summaries.
  10. Continuously seek opportunities for professional growth and development to enhance knowledge and skills in the field of hospital medicine.

Job Qualifications
  • Master's Degree In Nursing From An Accredited Nurse Practitioner Program

  • Current And Unrestricted Licensure As An Advanced Practice Registered Nurse (Aprn) In The State Of Employment

  • Board Certification As A Nurse Practitioner In Adult-Gerontology Or Family Practice

  • Minimum Of 2-3 Years Of Experience As A Nurse Practitioner In A Hospital Or Acute Care Setting

  • Strong Clinical Skills And Ability To Work Independently In A Fast-Paced And Dynamic Environment
